Thank you for your interest in the City EMT program. 
Please be advised of the following: There are three essay questions within the application and four documents that will need to be downloaded.  It is advised that applicants read the essay questions, formulate their answers in a word document and copy and paste.  Applicants should gather PDF files of the required four documents in preparation of submitting their application.  The four documents are:
​                                                                     1) Valid Identification or Driver License
                                                                     2) High School Diploma or GED
                                                                     3) Current Resume
                                                                    4) Copy of Completed COVID Vaccination Card
                                                                    5) High School Transcripts (can be unofficial copy)
                                                                    6) (optional) College Transcripts (can be unofficial copy)
